State-Based Perinatal Quality Collaboratives

 

The purpose of this program is to enhance state-based PQCsÂ’ ability to collect timely, high quality data in order to provide feedback to member facilities to improve perinatal care. The goal is to provide support for current efforts and to include higher proportions of participating hospitals in their states, as well as expand their current projects to address a broader range of neonatal and maternal health issues.

Applicant Eligibility
A. Eligibility Information 1. Eligible Applicants: State Health Departments or their Bona Fide Agents (this includes the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau). A Bona Fide Agent is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application. 2. Special Eligibility Requirements: For Module A, eligibility is limited to the State Health Departments (or their Bona Fide Agent) that are not currently funded under CDC-RFA- DP11-1110, and that have demonstrated participation in an ongoing Perinatal Quality Collaborative, with an existing statewide collaborative project with established pediatric/neonatology and obstetric/perinatology leadership. For Module B, eligibility is limited to existing Perinatal Quality Collaborative Grantee states currently funded under CDC-RFA- DP11-1110. Applicants eligible to apply for this module are Leland Stanford Junior University (acting as a bona fide agent of the California Department of Health), Health Research, Inc. (acting as a bona fide agent of the New York State Department of Health, and Ohio Department of Health. 3. Justification for Less than Maximum Competition: Competition for this funding will be limited to State Health Departments or their Bona Fide Agents who have demonstrated participation in an ongoing Perinatal Quality Collaborative, with an existing statewide collaborative project with established pediatric/neonatology and obstetric/perinatology leadership (Module A). States that are currently funded under CDC-RFA- DP11-1110 are not eligible for Module A. State Health Departments or their Bona Fide Agents are designated as registration areas for vital statistics and are uniquely qualified to perform the programmatic activities for this project. Moreover, they have the ability and authority to obtain population-based information from core data sets (such as birth and death files), which is crucial for the successful functioning of a statewide collaborative.
