Preventing Heart Attacks and Strokes in High Need Areas
The purpose of this one-year funding announcement is for the MS Delta Health Collaborative (MDHC) to complete existing activities that aim to prevent heart disease, stroke, and related chronic diseases, and would otherwise not be done because of limited time and funds. This program will continue to intensify collaboration among the Mississippi Department of Health, existing chronic disease programs (e.g., heart disease and stroke prevention, diabetes, tobacco, nutrition, physical activity), local health departments, other community health care providers (e.g., Federally Qualified Health Centers), local communities, and the CDC. MDHC will continue to implement interventions that focus on priority populations across Mississippi's 18-county Delta Region and address "ABCS" (i.e., Aspirin therapy, Blood pressure control, Cholesterol management, Smoking cessation, and Smoke-free ordinances). MDHC will continue to address and/or complete seven key recipient activities: 1) Program Infrastructure, Staffing, Management and Support; 2) Fiscal Management; 3) Leadership Team, Partnership Development, and Maintenance; 4) Implementation of Clinical Interventions; 5) Implementation of Community-Based Educational, Policy, and Systems Interventions; 6) Program Evaluation and Surveillance; and 7) Collaboration with other Chronic Disease Programs.

Applicant Eligibility
This is a Type 2 continuation cooperative agreement, which will provide funds for one additional budget year (from September 30, 2015 to September 29, 2016) beyond the previously approved project period (from September 30, 2010 to September 29, 2015) for DP10:1013 Mississippi Delta Health Collaborative. As such, this award will be a continuation of funds intended only for the grantee previously awarded under DP10-1013. Accordingly, financial assistance from this FOA will be provided only to the Mississippi Department of Health / Mississippi Delta Health Collaborative (MDHC). No other applications will be solicited, or will be reviewed.
