Partner Support for Heart Disease and Stroke Prevention

 

This project aims to enhance the dissemination and promotion of evidence-based strategies and resources to support the prevention and control of heart disease, stroke, and related risk factors such as high blood pressure, high LDL cholesterol, and high sodium intake. The grantee will help strengthen, improve, and sustain collaborations and partnerships at the state and local levels across the public and private sectors. The grantee will: Disseminate consistent, evidenced-based practices, tools, strategies, and cardiovascular disease (CVD) messages; Create and facilitate opportunities for peer learning and sharing of best practices and innovative approaches in cardiovascular disease prevention among stakeholders at the local level (i.e., convene partners, network sharing site for local partners); Sustain the momentum of Million Hearts beyond 2017 by helping maintain key partnerships and effectively communicate the impact of Million Hearts, highlighting successes and lessons learned from the initiative; Provide insight into current and future trends in cardiovascular disease treatment, care and prevention; Utilize network members, affiliates, and/or chapters to facilitate community-clinical linkages at the state and local level, and to bring faces and stories that describe the impact of efforts on cardiovascular health to the national level. Package and promote CDC’s cardiovascular tools and products to relevant stakeholder audiences.
