Affordable Care Act (aca) Nurse-managed Health Clinics
To provide federal funding to support the development and operation of Nurse-Managed Health Clinics (NMHC) to: 1) improve access to primary health care, disease prevention, and health promotion in medically underserved areas (including enhancements of outreach strategies); 2) enhance nursing practice by increasing the number of structured clinical teaching sites for undergraduate and graduate nursing students; and 3) enhance electronic processes for establishing effective patient and workforce data collection systems. Under this program, the focus supports the training and practice development sites for nurse practitioners to build the capacity of the primary care provider workforce.

Who is eligible to apply/benefit from this assistance?
Applicant Eligibility
Eligible applicants must be nurse-practice arrangements managed by advanced practice nurses who provide primary care or wellness services to underserved or vulnerable populations and are associated with a school, college, university or department of nursing, federally qualified health center or independent nonprofit health or social services agency or Federally Recognized Indian Tribal Government and Native American Organization.

Any Nurse Managed Clinic with undergraduate or graduate nursing students receiving clinical education or experiences during this grant period must provide a Memorandum of Agreement with the school(s) of nursing for clinical placement arrangement. This program is excluded from coverage under OMB Circular No. A-87.

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions.

How are proposals selected?
Competing applications are reviewed by non-Federal expert consultant(s) for technical merit recommendations. Approval of an application will be based on analysis of the factors set forth in the applicant’s proposal. See the section on Application Review in the funding opportunity announcement. An administrative preference is available for applicants affiliated with a university, college, or department of nursing. The NMHCs must ensure that a minimum of 30 percent of their training slots are available for advanced practice nursing students, and 100 percent must be available for training for primary/ community health care provider students. The program has a funding preference that shall be granted to applicants with projects that will substantially benefit rural or underserved populations or help meet public health nursing needs in State or local health departments.

Awards may be used to support the operation of nurse managed centers, to enhance electronic data collection systems, to support training programs for nurse practitioner students and students of other health related disciplines, and to support the enhancement of patient care and accessibility.
What are the requirements after being awarded this opportunity?
Reporting
The awardee will be required to submit performance and progress reports as well as status-federal financial reports (see the program announcement and notice of award for details for each required report). The awardee must submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System within 30 days of the end of each calendar quarter. A Federal Financial Report (SF-425) according to the following schedule: http://www.hrsa.gov/grants/manage/technicalassistance/federalfinancialreport/ffrschedule.pdf. A final report is due within 90 days after the project period ends. If applicable, the awardee must submit a Tangible Personal Property Report (SF-428) and any related forms within 90 days after the project period ends. New awards (“Type 1”) issued under this funding opportunity announcement are subject to the reporting requirements of the Federal Funding Accountability and Transparency Act (FFATA) of 2006 (Pub. L. 109–282), as amended by section 6202 of Public Law 110–252, and implemented by 2 CFR Part 170. Grant and cooperative agreement recipients must report information for each first-tier subaward of $25,000 or more in federal funds and executive total compensation for the recipient’s and subrecipient’s five most highly compensated executives as outlined in Appendix A to 2 CFR Part 170 (FFATA details are available online at http://www.hrsa.gov/grants/ffata.html). Competing continuation awardees, etc. may be subject to this requirement and will be so notified in the Notice of Award. Cash reports are not applicable. Progress reports are not applicable. Expenditure reports are not applicable. Performance monitoring is not applicable.
Auditing
In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.
Records
Grantees are required to maintain grant accounting records three years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the three year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular three year period, whichever is later.
